Community Guide and Community Engagement are person-centered services that are individually tailored for every person’s unique goals and preferences.

Community Guide

Basic Plus waiver or the Core waiver participants can develop skills to take part in community activities through individualized, short-term services.

Community Engagement

Individual and Family Services (IFS) waiver and Pre-Admission Screening Residential Review (PASRR) program participants can engage in one-on-one, long-term services that promote community integration.

Take Aim’s Community Guide and Community Engagement professionals will:

  • Partner with individuals to meet their self-identified community integration goals.

  • Connect people with resources.

  • Collaborate with community members and leadership to create new opportunities.

  • Provide a flexible atmosphere that will ensure participation in activities that promote independence and personal growth.
